The AAPC CPC exam tests 150 questions across anatomy, ICD-10-CM, CPT, HCPCS Level II, and coding guidelines in a 4-hour open-book format. A 90-day plan gives most beginners enough runway to build real competence rather than last-minute cramming.

Days 1–30: Build the Foundation. Spend the first month on medical terminology and anatomy by body system — this underpins everything else. Pair each anatomy topic with the related ICD-10-CM chapter so you're learning diagnosis coding in context, not in isolation. Aim for one body system every 4–5 days, with daily flashcard review for terminology recall.

Days 31–60: Master CPT and HCPCS. Move into CPT coding section by section — Evaluation & Management first since it appears most frequently on the exam, followed by Surgery, Radiology, Pathology/Lab, and Medicine. Practice tabbing your codebook here; index navigation speed matters as much as knowledge during the timed exam. Layer in HCPCS Level II alongside CPT, since supplies and DME codes often pair with the same encounters.

Days 61–75: Full-Length Mock Exams. Take at least 3–4 full 150-question timed mock exams under real exam conditions. Review every wrong answer by tracing back to the guideline or index entry you missed, not just memorizing the correct code. This is where most candidates discover their weak areas — usually modifiers, bundling rules, or E/M leveling.

Days 76–90: Targeted Revision. Use your mock exam data to focus only on weak chapters. Re-tab your codebooks, review official AAPC guideline notes, and take one final timed mock 3–4 days before your scheduled exam date — then rest. Walking in fresh matters more than squeezing in one more late-night session.

For aspiring coders and billers, understanding these shifts isn't optional — it shapes where the jobs actually are.

Automation is the top investment priority. Industry research shows automation and AI now rank as the single biggest RCM investment priority for 2026, covering everything from payer analytics to coding support and prior authorization tools. This doesn't eliminate coding jobs — it shifts them toward review, audit, and exception-handling roles that still require trained human coders to validate AI-suggested codes.

Denials are a growing pain point. A large share of hospitals are reporting bad debt levels running into the millions, largely tied to payment collection and denial issues. This is pushing healthcare organizations to invest heavily in denial prevention and appeals expertise — making denial management one of the fastest-growing specialization tracks within RCM.

Outsourcing keeps expanding toward India. Nearshore and offshore RCM outsourcing continues to grow as US providers look for scalable, cost-effective support for high-volume, judgment-heavy processes like eligibility checks, coding, and billing follow-up. This sustains strong demand for AAPC-certified coders and billers based in India, particularly those trained on current payer rules and denial workflows.

Regulatory changes are reshaping reimbursement. New legislative reforms are altering coverage rules and increasing patient-pay balances, which means billing teams need sharper skills in patient financial communication and flexible payment models — not just claims submission.

For students, the takeaway is simple: foundational coding knowledge still matters, but pairing it with denial management, payer-specific rules, and comfort working alongside AI tools will define the most resilient RCM careers over the next few years.

CMS released the FY2026 ICD-10-CM update effective October 1, 2025, and it's a substantial one: 487 new diagnosis codes, 38 code revisions, and 28 deletions, alongside extensive updates to the official coding guidelines. Coders who haven't reviewed these changes risk claim denials simply from using outdated or deleted codes.

Greater specificity for pain reporting. Sixteen new R-codes were added to improve specificity for pelvic, perineal, subpubic, abdominal, and flank pain and tenderness, with R10.2 (pelvic and perineal pain) now converted into a parent code supporting more granular child codes. Five additional codes were created for reporting costovertebral angle tenderness with greater detail.

New code for cannabis hyperemesis syndrome. A newly created code, R11.16, allows coders to specifically document cannabis hyperemesis syndrome — a cyclic vomiting condition seen in long-term cannabis users — rather than relying on nonspecific nausea/vomiting codes as before.

Expanded social determinants of health (SDoH) codes. Reflecting the industry's growing focus on SDoH, the update converts Z59.86 (financial insecurity) into a parent code with new child codes, and adds new Z77.3 codes for documenting exposure to war theater. Food allergy reporting also gained specificity, with both milk and egg allergy codes expanded into more granular options.

New remission code for type 2 diabetes. A new code, E11.A, lets coders document type 2 diabetes mellitus without complications in remission — based on provider documentation — closing a long-standing gap in diabetes status reporting.

Significant guideline revisions. Chapter 1 guidelines for HIV coding saw one new guideline added and eight others revised, mostly clarifying sequencing rules between HIV disease (B20) and asymptomatic HIV status (Z21). Chapter 12 introduced over 100 new codes for non-pressure chronic ulcers, now classified with much greater anatomical and severity detail.
